How should this issue be escalated? Most organizations don't document decisions.They document processes. But processes only explain what should happen. Decisions explain why. AI needs both.

Walk through almost any organization and you'll hear the same phrases."Ask Susan.""Mike knows how that works.""It depends.""We've always done it this way." Those aren't just conversations. They're indicators. They reveal that important knowledge still lives inside people instead of inside the organization. For decades...That worked.Today...It doesn't scale. Not for new employees.
